If you have another admin account on your Mac, sign into that one and reset your password from there. This is the fastest solution. If you only have one account, use Recovery Mode to create a new admin account and regain control.

- Trying to use your Apple ID password instead of your Mac admin password. They are two different passwords
- Not holding Command-R long enough. Hold it for a full 5-10 seconds after pressing power
- Shutting down and restarting without waiting. Wait 10 seconds after shutdown to give the Mac time to fully power down
